For those clinics that have 1 - 5 physicians it makes sense to have a hosted desktop.  Eliminate the need for purchasing fat PC desktops, hardware repair costs, software costs, loss of data, security, IT management.  The ability to move from exam room to exam room and anywhere in the clinic with the use of a smart card.  Making your staff efficient and having patient information at your finger tips instantly rather then logging off and on which wastes your time and increases patient wait time.  With the hosted EMR desktop you have a choice from Mac, Linux, Windows XP, Vista and Windows 7; be able to use it at the clinic or securely from home following up on your patients results.  We offer this service to Canada, USA & Eastern Europe and if are interested in getting this in your clinic please visit http://www.greendatacentre.ca

I know of a facility in Southern Minnesota (USA) that is 100% hosted VMWare environment for cliinical PCs.  Even given the costs of SANs, Servers, Fiber switching, etc... they saved ~30% over going with traditional fat clients.  Their implementation is ~400 end user stations. 

They have some huge advantages in security and user context too.  The desktop that you sign in with at the beginning of the day travels with you everywhere!  They use a LeoStream broker to move the desktops to where ever you authenticate.  It is VERY slick and the providers love it.

I think virtualization is awesome!  We use VMWare ESX to post servers and desktops all over the country.  Doctors can VPN in to get to the web pages and then RDP straight to their machines.  It is very cool.

In the simplest of terms, your computer travels with you.  Anywhere you log into the broker your specific desktop travels to you.  No more different apps on different machines, etc.  Everything you have on your system is completely portable to any other physical location.
