Do you have a PCMH pilot going on?  What are you learning?  Are you seeing better clinical outcomes?

What exactly is Patient Centered Medical Home? Has it really become a defined thing yet?

A Patient Centered Medical Home (PCMH) is a team, headed by a personal physician who offers coordinated and permanent care during the lifetime of a patient to make the most out of health outcomes.

It is responsible for taking care of all the health care concerns of the patient and arranging appointments with professional, when required. The PCMH includes the stipulation of preventive services, treating any chronic or severe illness and helping with any health issues. They work together to provide high quality care, communication, corporation and security.
